Optional anchor elements

Use the “optional anchor” hierarchical relationship to model any optional anchor elements (the inverse is “optional anchor in”.)

Use the Ordered List of the context to model where in the sequence the optional anchor occurs (or looking at it the other way, where the anchor may or may not occur).
